Please specify the names of your children and age. If you DO NOT want your school age child sitting at a kids table, please indicate that next to their names.
We will do our best to accommodate but please Indicate a few options, if possible.
Please indicate if there are any special dietary requirements for anyone in your reservation (aside from Kosher, of course!).
In order to keep costs down, we count on sponsorships. All donations are tax deductible. Please indicate your donation amounts. The list of Purim sponsors will  will be publicized, unless otherwise indicated.
